Updates on Billing, PSC Implementation, and the FHO+ Transition

Dear SGFP Members,
Over the past several months, we’ve committed to keeping you informed and supported through the changes affecting your practices from the latest implementation details of the Physician Services Agreement, including billing and fee updates, and the transition to FHO+. This week, Dr. Salesh Budhoo, SGFP Tariff Lead, and I are writing together to provide an update, to acknowledge the pressure many of you are feeling and to share resources to support you.
“Let’s Talk Billing”: Billing Support for Every Member
Our popular Let’s Talk Billing sessions continue and are open to all members across family medicine payment models. These monthly sessions are designed to support your day-to-day work, with:
- Dedicated time for your billing questions
- Practical examples you can apply immediately
- Clinical highlights from focused practices to help optimize billing opportunities

PSC Implementation Highlights
- Year 3 (2021-24 PSA) and year 1 (2024-2028 PSA) increases are being implemented permanently on April 1, 2026.
- Relativity changes on your current RA will no longer be shown as they will be included in the permanent changes.
- You will see temporary relativity-adjusted changes for years 2-4 beginning on April 1, 2026. Year 2-4 changes will be permanent on April 1, 2027.

FHO+ Transition: What We’re Hearing and What We’re Working On
FHO+ go-live is now less than two weeks away, and we want to acknowledge the anxiety many of you are feeling. This transition is happening at a moment when burnout is high and you are already stretched, and it means rethinking processes that have been stable for your practice. Your colleagues at the SGFP, including both of us writing this letter, feel that pressure too. Is FHO+ perfect? No. But no payment model is for all the ways in which we practice. Is it a good step forward? Yes, it addresses the pain points we’ve heard for years, such as getting rid of negation, paying for your time spent on indirect care and administration.
We also know there are specific concerns for rural and northern physicians. We appreciate the excellent care provided by rural generalists and the commitment to taking care of all patients in their communities. FHO+ may have issues that were unintended, and the SGFP and OMA are looking at workable solutions for those affected. We will share updates as we have them.
Last week’s FHO+ webinar was attended by more than 2,000 members and generated over 600 questions. Several common themes emerged and we are sharing resources developed based on the questions you raised. The SGFP will continue to work with the OMA to ensure you get the resources you need.
